1.6. Why does our company have to collect and store your personal data?
The authorized persons of the operator process the personal data of the data subject for the purpose of providing extracurricular educational activities. Personal data are processed on a legal basis in accordance with § 13 par. 1 letter a) and b) of Act no. 18/2018 Z.z. We are committed to ensuring that the information we collect and use is appropriate for this purpose and does not infringe your privacy.

The Operator will process (collect, store and use) the provided information in a manner that is in accordance with Act no. 18/2018 Z.z. on the protection of personal data on the amendment of certain laws, Act no. 40/1964 Coll. Civil Code, Act no. 513/1991 Coll. Commercial Code and Act no. 311/2001 Coll. Labor Code as amended.

The company processes personal data in accordance with Act no. 395/2002 Coll. on archives and registries and on the amendment of certain laws as amended. The operator shall store the personal data of the data subject in accordance with the registration plan.

Whenever we own or process your personal data, you, the data subject, have the following rights:
– obtain confirmation from the controller whether personal data concerning you are being processed. If the controller processes such personal data, you, as the data subject, have the right to access this personal data and information on the purpose of the processing of personal data; categories of personal data processed; the identification of the recipient or of the category of recipient to whom the personal data have been or are to be provided, in particular the recipient in a third country or an international organization, if possible; the retention period of personal data, if this is not possible, information on the criteria for its determination; the right to request from the controller the correction of personal data concerning the data subject, their deletion or restriction of their processing, or the right to object to the processing of personal data.
– for the operator to correct incorrect personal data concerning you without undue delay. With regard to the purpose of the processing of personal data, you have the right as a data subject to supplement incomplete personal data.
– for the controller to restrict the processing of personal data if:
o as the data subject, you object to the accuracy of the personal data during a period allowing the controller to verify the accuracy of the personal data,
the processing of personal data is illegal and, as the data subject, you object to the deletion of personal data and instead request a restriction on their use, o the controller no longer needs personal data for the purpose of processing personal data, but you need them as a data subject to exercise a legal claim, or o you object to the processing of personal data according to § 27 par. 1 of Act no. 18/2018 Coll., Until it is verified whether the justified reasons on the part of the operator prevail over the justified reasons of you as the affected person.

1.12. The right of the person concerned to file a motion to initiate proceedings pursuant to para. § 100 of Act. No. 18/2018 Z. Z. On the protection of personal data and on the amendment of certain laws
An affected person who considers that his or her personal data has been processed or misused may file a petition with the Office for Personal Data Protection of the Slovak Republic (hereinafter the “Office”) to initiate proceedings on personal data protection. The motion to initiate proceedings may be submitted in writing, in person orally in the minutes, by electronic means, and must be signed with a guaranteed electronic signature, telegraph or fax, which must be added to the minutes in writing or orally within 3 days.
The proposal in question must, in accordance with the provisions of § 63 para. 2 of the Personal Data Protection Act include:
name, surname, address of permanent residence and signature of the petitioner, identification of the person against whom the petition is directed; name or surname, registered office or permanent residence, or legal form and identification number, subject of the proposal with an indication of which rights, according to the petitioner, were violated during personal data processing, evidence to support the allegations in the proposal, a copy of the document proving 28, if such a right could be exercised, or an indication of reasons worthy of special consideration.

1.13. How can you exercise your rights?
If you want to exercise your rights as a data subject in connection with the processing of your personal data, you can contact us at the email address skolaosteopatie@gmail.com. We will then send you a request for access to the subject. By completing and sending this request back to our e-mail address and subsequent identification, we, as the person concerned, on the basis of the information provided in the request, will register and process it within the legal deadline.
